OUR PROGRAM

Multidisciplinary Board Approval

Acquiring approval from our Multidisciplinary Board is the final step prior to scheduling your surgery. In order to send your file to the Multidisciplinary Board, you must have completed all of the following:

• All forms and questionnaires
• All pre-operative testing
• Medical Doctor Letter of Clearance
• Approval from your insurance company

Your file is then submitted to the multidisciplinary board which consists of the doctors in the following specialties:

• Cardiology
• Nutrition
• Dietary
• Pulmonary
• Psychiatry
• Social Work
• Respiratory

Also part of the board are Nurse Clinical Manager, Surgical Chairman and Surgical Administrator.

Once you have received final approval for your surgery from our office and from your insurance company, Julie Johnson will work with you to schedule your surgical procedure.
